The state's Advanced Directives for Health Care Decisions and Physician's Order for Life Sustaining Treatment (POLST) form will be presented and its differences explained. Each participant will receive a free copy of their state's Advanced Directives for Health Care Decisions and POLST for their use.

Making final arrangements can be overwhelming for those close to you. Clear choices may be difficult to make. When the death of a loved one occurs, there are many decisions that will have to be made in a very short period of time.
